Any other PPO insurance company can be billed, but they pay out-of-network benefits. If you have an insurance carrier other than those listed above, they have their own approved providers that they prefer be used. If you wish us to provide your care, we will have to call them to get prior authorization. Here is a list of some of these other carriers needing prior authorization to provide treatment -
